Contact form Medical-1 Pseudo element not displaying correctly

Hello, I am having a problem with the contact form in The Gem, Medical-1, in the "date" field where there is supposed to be a calendar icon or pseudo element that is not displaying correctly. Instead, I get "e635" and no icon (please see attached screen capture). I have tracked it down to the css; ../home/tnortho/public_html/wp-content/themes/thegem/css/thegem-new-css.css

...however, after copying the format for the "phone" field (which IS displaying correctly), I still could not get the "e365" to go away and be replaced by the correct pseudo element / icon.

Could you PLEASE instruct me as to how to edit the CSS to resolve this issue?

    Thank you!
